HO2S-21 (Bank 2 Sensor 1) Circuit Closed Loop

Possible Causes:



EVAP system leaks or restrictions, charcoal canister problems
 
EVAP system leaks or restrictions, charcoal canister problems
 
HO2S air reference hole plugged (check for dirt on the outside)
 
HO2S signal circuit is shorted to sensor or chassis ground
 
HO2S is damaged or contaminated due to improper fuel usage
 
PCM has failed

HO2S-21 (Bank 2 Sensor 1) Circuit Low Input

Possible Causes:



EVAP system leaks or restrictions, charcoal canister problems
 
Fuel system too lean (possible low fuel pressure, water in fuel)
 
HO2S air reference hole plugged (check for dirt on the outside)
 
HO2S signal circuit is shorted to sensor or chassis ground
 
HO2S is damaged or contaminated due to improper fuel usage
 
PCM has failed

Possible Causes:



Air leaks in the exhaust system, intake manifold, vacuum lines
 
Fuel system too rich (fuel pressure too high, fuel pressure regulator leaking, or one or more fuel injectors sticking/leaking)
 
HO2S element is silicon, water or fuel contaminated
 
HO2S signal tracking (water intrusion) in the connector causing a short between the HO2S signal and heater power circuits
 
HO2S low reference circuit is open or shorted to power (B+)
 
PCM has failed

HO2S heater element is damaged or has failed
 
PCM has failed

Fuel Trim System Lean (Bank 1)

Possible Causes:



Air leaks in intake manifold, exhaust pipes or exhaust manifold
 
Fuel control sensor is out of calibration (ECT, IAT or MAF)
 
Low fuel pressure (fuel filter clogged, pressure regulator failure)
 
One or more injectors restricted or pressure regulator has failed
 
HO2S element is contaminated, deteriorated or has failed
 
Vacuum hose is disconnected, broken, leaking or loose

Fuel Trim System Rich (Bank 1)

Possible Causes:



Base engine "mechanical" fault affecting one or more cylinders
 
Excess fuel vapors in crankcase (the oil needs to be changed)
 
EVAP system component has failed or canister fuel saturated
 
Fuel control sensor is out of calibration (i.e., ECT, IAT or MAF)
 
Fuel delivery system supplying too much fuel during cruise or idle periods (e.g., faulty fuel pump, or faulty pressure regulator)
 
Fuel injector(s) is leaking or stuck partially open (one or more)
 
HO2S is contaminated, deteriorated or it has failed

Possible Causes:



Air leaks in intake manifold, exhaust pipes or exhaust manifold

APP2 signal circuit is shorted to chassis or sensor ground

TP Sensor 5v VREF circuit is open or shorted to ground
 
PCM is damaged (check the power ground circuit resistance)

APP Sensor 2 signal circuit is shorted to the 5v VREF circuit
