CV solenoid is stuck partially or fully open or filter is plugged

Fuel filler cap stuck closed (vacuum relief cannot occur)
 
Contaminated fuel vapor elbow at the EVAP canister, or the EVAP canister is restricted or canister purge valve stuck open
