Possible Causes:



Faulty or improperly installed output shaft (countershaft) speed sensor
 
Poor connections and loose terminals at the output shaft (countershaft) speed sensor and the PCM
 
"Open" in the wires between PCM connector terminals and ground or poor ground
 
"Open" or "Short" in the wire between PCM connector terminal and the output shaft (countershaft) speed sensor connector
 
PCM needs to be updated with the latest software
