Possible Causes:



Fuel pressure regulator leaking or fuel injectors leaking
 
HO2S signal circuit is shorted to the heater power circuit
 
HO2S may be contaminated or it has failed
 
HO2S heater is damaged or has failed
 
PCM has failed

HO2S-12 (Bank 1 Sensor 2) Slow Response

Possible Causes:



HO2S signal circuit is open or shorted to ground
 
HO2S element is contaminated or it has failed
 
HO2S heater is damaged or has failed
 
Intake air leaks, exhaust manifold leaks or PCV system leaks
 
MAF sensor out of calibration (it may be dirty or contaminated)
