Trouble Code: P0011

Intake Valve Timing Control Range/Performance (Bank 1):

Trouble Code Conditions: Engine started, engine running at idle speed, followed by a quick and steady acceleration in 1st gear to over 2000 rpm, and the PCM detected a problem in the operation of the IVT control during testing.

Possible Causes:



Check for an accumulation of debris on the CMP sensor pickup
 
CKP POS sensor or CMP Phase sensor signal is erratic
 
IVT connector is damaged or loose (an intermittent fault)
 
IVT solenoid control circuit is open or shorted to ground
 
IVT solenoid is damaged or has failed
 
PCM has failed
 

Trouble Code: P0021

Intake Valve Timing Control Performance Bank 2:

Trouble Code Conditions: Engine started, engine running at idle speed, followed by a quick and steady acceleration in 1st gear to over 2000 rpm, and the PCM detected a problem in the operation of the IVT control during testing.

Trouble Code Conditions: Engine started, system voltage from 10.5-16.0v, and the PCM detected the HO2S-11 heater control circuit input (voltage) was excessively low (i.e., it was operating out of its normal range).

Possible Causes:



HO2S heater control connector is damaged or loose (open)
 
HO2S heater power circuit is open (check the No. 8 fuse 15A)
 
HO2S heater assembly is damaged or has failed
 
PCM has failed

HO2S-11 (Bank 1 Sensor 1) Heater Circuit High Input:

Trouble Code Conditions: Engine started, system voltage from 10.5-16.0v, and the PCM detected the HO2S-11 heater control circuit input (voltage) was excessively high (i.e., it was operating out of its normal range).

Possible Causes:



HO2S heater control connector is damaged or loose (shorted)
 
HO2S heater control circuit is shorted to ground
 
HO2S heater assembly is damaged or has failed
 
PCM has failed

HO2S-12 (Bank 1 Sensor 2) Heater Circuit Low Input:

Trouble Code Conditions: Engine started, system voltage from 10.5-16.0v, and the PCM detected the HO2S-12 heater control circuit input (voltage) was excessively low (i.e., it was operating out of its normal range).

Possible Causes:



HO2S heater control connector is damaged or loose (open)
 
HO2S heater power circuit is open (check the No. 8 fuse 15A)
 
HO2S heater assembly is damaged or has failed

HO2S-12 (Bank 1 Sensor 2) Heater Circuit High Input:

Trouble Code Conditions: Engine started, system voltage from 10.5-16.0v, and the PCM detected the HO2S-21 heater control circuit input (voltage) was excessively low (i.e., it was operating out of its normal range).

Possible Causes:



HO2S heater control connector is damaged or loose (open)

HO2S heater control connector is damaged or loose (open)

HO2S-22 (Bank 2 Sensor 2) Heater Circuit High Input:

Trouble Code Conditions: Engine started, system voltage from 10.5-16.0v, and the PCM detected the HO2S-22 heater control circuit input (voltage) was excessively high (i.e., it was operating out of its normal range).

Possible Causes:



HO2S heater control connector is damaged or loose (shorted)
 
HO2S heater control circuit is shorted to ground
 
HO2S heater assembly is damaged or has failed
 
PCM has failed
 

Trouble Code: P0101

Mass Airflow Sensor Signal Range/Performance:

Trouble Code Conditions: Engine started, engine running at light engine load, and the PCM detected the MAF sensor signal was excessively high; or with the engine running under high engine load, the PCM detected the MAF sensor signal was excessively low under these operating conditions.

Possible Causes:



Harness or connectors (The sensor circuit is open or shorted.)
 
Intake air temperature sensor
 
Mass air flow sensor
 
Intake air leaks
 

Trouble Code: P0102

Mass Air Flow Sensor Circuit Low Input:

Trouble Code Conditions: Key on or engine running; and the PCM detected an unexpected low voltage condition on the MAF sensor signal circuit in the CCM test.

Possible Causes:



MAF sensor connector is damaged, loose or shorted
 
MAF sensor signal circuit is shorted to ground
 
MAF sensor power circuit is open (check for power from relay)
 
MAF sensor is damaged or has failed
 
PCM is damaged
 

Trouble Code: P0103

Mass Air Flow Sensor Circuit High Input:

Trouble Code Conditions: Key on or engine running; and PCM detected an unexpected high voltage condition on the MAF sensor signal circuit in the CCM test.

Possible Causes:



MAF sensor signal circuit is open (check for loose connector)
 
MAF sensor is shorted to VREF or system power
 
MAF sensor is damaged or has failed
 
PCM is damaged
 

Trouble Code: P0107

Absolute Pressure Sensor Circuit Low Input:

Trouble Code Conditions: Key on or engine running; and the PCM detected an unexpected low voltage on the Absolute Pressure signal circuit during the CCM test. Note: This signal is used during several of the Main Monitor tests.

Possible Causes:



Absolute pressure sensor signal circuit is shorted to ground
 
Absolute pressure sensor is damaged or has failed
 
PCM has failed
 

Trouble Code: P0108

Absolute Pressure Sensor Circuit High Input:

Trouble Code Conditions: Key on or engine running; and the PCM detected an unexpected high voltage on the Absolute Pressure signal circuit in the CCM test. Note: This signal is used during several of the Main Monitor tests.

Possible Causes:



Absolute pressure sensor connector is damaged (open circuit)
 
Absolute pressure sensor signal circuit open, shorted to VREF
 
Absolute pressure sensor ground circuit is open
 
Absolute pressure sensor is damaged or has failed

Intake Air Temperature Sensor Circuit Low Input:

Trouble Code Conditions: Key on or engine running; and the PCM detected an unexpected low voltage condition on the IAT sensor signal circuit in the CCM test.

IAT sensor connector is damaged (it may be shorted)
 
IAT sensor signal circuit is shorted to ground
 
IAT sensor is damaged or has failed

Trouble Code Conditions: Key on or engine running; and PCM detected an unexpected high voltage condition on the IAT sensor signal circuit in the CCM test.

Trouble Code Conditions: Key on or engine running; and the PCM detected an unexpected low voltage condition on the ECT sensor signal circuit in the CCM test.

ECT sensor signal circuit is shorted to ground
 
ECT sensor is damaged or has failed
 
PCM has failed

Engine Coolant Temperature Sensor Circuit High Input:

Trouble Code Conditions: Key on or engine running; and PCM detected an unexpected high voltage condition on the ECT sensor signal circuit in the CCM test.

Possible Causes:



ECT sensor signal circuit is open (the connector may be loose)
 
ECT sensor ground circuit is open (connector may be loose)
 
ECT sensor signal circuit is shorted to sensor VREF
 
ECT sensor is damaged or has failed
 
PCM has failed
 

Trouble Code: P0121

Throttle Position Sensor Signal Range/Performance

Trouble Code Conditions: DTC P0510 not set, engine started, and the PCM detected a high voltage condition on the TP sensor signal circuit under low engine load conditions, or the PCM detected a low voltage condition on the TP sensor signal circuit under high engine load conditions.

Possible Causes:



TP sensor connector is damaged or loose (intermittent fault)
 
TP sensor signal circuit is open, shorted to ground or to VREF
 
TP sensor is damaged
 
PCM has failed
 

Trouble Code: P0122

Throttle Position Sensor Circuit Low Input:

Trouble Code Conditions: DTC P0510 not set, engine started, system voltage over 10.0v, and the PCM detected an unexpected low voltage condition on the TP sensor signal during the CCM test.

Possible Causes:



TP sensor connector is damaged (possible intermittent short)
 
TP sensor signal circuit is shorted to ground
 
TP sensor is damaged
 
PCM has failed
 

Trouble Code: P0123

Throttle Position Sensor Circuit High Input

Trouble Code Conditions: DTC P0510 not set, engine started, system voltage over 10.0v, and the PCM detected an unexpected high voltage condition on the TP sensor signal during the CCM test.

Possible Causes:



TP sensor connector is damaged or loose (intermittent open)
 
TP sensor signal circuit is open or shorted to VREF
 
TP sensor is damaged
 
PCM has failed
 

Trouble Code: P0125

ECT Sensor Excessive Time To Enter Closed Loop:

Trouble Code Conditions: DTC P0115 not set, engine started, engine runtime over 6 minutes, ECT sensor more than 176ºF, and the PCM detected that the engine did not achieve closed loop operation during the test period. Note: If DTC P0125 is set with DTC P0115, repair DTC P0115 first!

Possible Causes:



Check the operation of the thermostat (it may be stuck open)
 
ECT sensor signal circuit has a high resistance condition
 
ECT sensor is damaged or has failed (out of calibration)
 

Trouble Code: P0127

Intake Air Temperature Sensor Range/Performance:

Trouble Code Conditions: Engine started cold, vehicle driven at a speed of over 45 mph for 10 minutes, and the PCM detected the IAT sensor input was not valid when compared to the change in the ECT sensor signal in the test.

Possible Causes:



IAT sensor connector is damaged or loose (intermittent fault)
 
IAT sensor is damaged or has failed
 
PCM has failed
 

Trouble Code: P0128

Thermostat function:

Trouble Code Conditions: The engine coolant temperature does not reach to specified temperature even though the engine has run long enough.

Possible Causes:



Check the operation of the thermostat (it may be stuck open)
 
Leakage from sealing portion of thermostat
 
Engine coolant temperature sensor
 

Trouble Code: P0130

HO2S-11 (Bank 1 Sensor 1) Closed Loop Fuel Control:

Trouble Code Conditions: Engine started, and after the vehicle was driven at a speed of 30-55 mph for 2 minutes, the PCM detected the HO2S signal voltage was excessively high or low, or it detected the HO2S signal did not reach the maximum or minimum voltages, or it took too long for the HO2S signal to switch from rich-to-lean or from lean-to-rich during the test.

Possible Causes:



HO2S signal circuit is open or shorted to ground
 
HO2S is damaged or it has failed
 
PCM has failed
 

Trouble Code: P0131

HO2S-11 (Bank 1 Sensor 1) Circuit Low Input (Lean Shift):

Trouble Code Conditions: Engine running in closed loop at a speed of over 20 mph for at least 20 seconds, IAT sensor signal from 14-122ºF, fuel level over 25%, and the PCM detected the HO2S-11 input did not reach a maximum voltage level of 0.60v, or it remained fixed at approximately 300 mv.

Possible Causes:



Low fuel pressure, fuel filter restricted or fuel injectors plugged
 
HO2S heater is damaged or has failed
 
HO2S is contaminated or it has failed
 
PCM has failed
 

Trouble Code: P0132

HO2S-11 (Bank 1 Sensor 1) Circuit High Input (Rich Shift):

Trouble Code Conditions: Engine started, IAT sensor signal from 14-122ºF, fuel level over 25%, vehicle driven at over 20 mph while in closed loop, and the PCM detected the minimum HO2S-11signal was more than 600 mv.

Possible Causes:



Fuel pressure regulator leaking or fuel injectors leaking
 
HO2S connector is damaged or loose (intermittent open circuit)
 
HO2S signal circuit is open or shorted to power
 
HO2S is contaminated or it has failed
 

Trouble Code: P0133

HO2S-11 (Bank 1 Sensor 1) Slow Response:

Trouble Code Conditions: Engine started, vehicle driven in Drive to over 50 mph at 1200-3100 rpm, IAT sensor signal from 14-122ºF, Fuel Schedule from 2.5-12.0 ms, and the PCM detected the average HO2S-11 response time indicated more than 1 second.

Possible Causes:



HO2S signal circuit is open or shorted to ground
 
HO2S element is contaminated, or HO2S heater has failed
 
Intake air leaks, exhaust manifold leaks or PCV system leaks
 
MAF sensor out of calibration (it may be dirty or contaminated)
 

Trouble Code: P0134

HO2S-11 (Bank 1 Sensor 1) Insufficient Activity Detected:

Trouble Code Conditions: Engine started, vehicle driven at over 20 mph while in closed loop, IAT sensor signal from 14-122ºF, and the PCM detected the HO2S signal was fixed under 300 mv, or the HO2S signal was fixed over 600 mv for 10 seconds, or it detected that the HO2S-11 input switched from rich to lean less than 5 times in a 10 second period.

Possible Causes:



HO2S heater is damaged or has failed
 
HO2S signal circuit is open or shorted to ground
 
HO2S is damaged, has fuel contamination or has failed

Trouble Code: P0135

HO2S-11 (Bank 1 Sensor 1) Heater Circuit Malfunction:

Trouble Code Conditions: Engine started, engine running in closed loop at under 3000 rpm, and the PCM detected an unexpected voltage condition (either too high or too low a voltage) on the HO2S-11 heater circuit.

Possible Causes:



HO2S heater control circuit is open or shorted to ground
 
HO2S heater control circuit is shorted to power
 
HO2S heater is damaged or has failed
 
PCM has failed
 

Trouble Code: P0137

HO2S-12 (Bank 1 Sensor 2) Circuit Low Input (Lean Shift):

Trouble Code Conditions: Engine running in closed loop at a speed of over 20 mph for at least 20 seconds, IAT sensor signal from 14-122ºF, fuel level over 25%, and the PCM detected the HO2S signal did not reach a maximum voltage level of 0.60v, or it remained fixed at approximately 300 mv.

Possible Causes:



Low fuel pressure, fuel filter restricted or fuel injectors plugged
 
HO2S may be contaminated or it has failed
 
HO2S heater is damaged or has failed
 

Trouble Code: P0138

HO2S-12 (Bank 1 Sensor 2) Circuit High Input (Rich Shift):

Trouble Code Conditions: Engine started, IAT sensor signal from 14-122ºF, fuel level over 25%, vehicle driven at over 20 mph for 2-3 minutes, and the PCM detected the HO2S-12 input did not reach a maximum voltage level of 0.60v, or the signal remained fixed at approximately 300 mv.

Fuel pressure regulator leaking or fuel injectors leaking
 
HO2S connector is damaged or loose (an open circuit)

HO2S-12 (Bank 1 Sensor 2) Slow Response

HO2S-12 (Bank 1 Sensor 2) Insufficient Activity Detected:

Trouble Code Conditions: Engine started, IAT sensor signal from 14-122ºF, fuel level over 25%, engine running at over 20 mph in closed loop, and the PCM detected the HO2S signal was fixed under 300 mv or over 600 mv, or it switched from rich to lean less than 5 times in 10 seconds.

Possible Causes:



HO2S heater is damaged or has failed
 
HO2S signal circuit is open or shorted to ground
 
HO2S element is contaminated, damaged or it has failed

Trouble Code: P0141

HO2S-12 (Bank 1 Sensor 2) Heater Circuit Malfunction:

Trouble Code Conditions: Engine running in closed loop at less than 3000 rpm, and the PCM detected an unexpected voltage on the HO2S heater circuit. Note: The current level of the HO2S circuit was too high or too low.

Possible Causes:



HO2S heater control circuit is open or shorted to ground
 
HO2S heater control circuit is shorted to power
 
HO2S heater is contaminated, damaged or it has failed
 
PCM has failed

HO2S-21 (Bank 2 Sensor 1) Closed Loop Fuel Control:

Trouble Code Conditions: Engine started, vehicle driven at a speed of 30-55 mph for 2 minutes, and the PCM detected the HO2S signal was excessively high or low, or it detected the HO2S signal did not reach the maximum or minimum voltages, or it took too long for the HO2S signal to switch from rich to lean.
