Transmission 1st Gear Ratio Incorrect

Possible Causes:



ATF level is too low, or the ATF is badly deteriorated
 
A/T line pressure is low, or the oil pump has failed
 
SSA, SSB, SSC or the PCS is stuck (mechanical fault)
 
A/T 1-2 clutch is slipping (mechanical fault)
 
A/T 1-2 shift valve is stuck (mechanical fault)
 
A/T Pressure regulator or the pressure modifier valve is stuck
 
A/T solenoid reducing valve is stuck (mechanical fault)
 
PCM has failed

Possible Causes:



DLC connector or the related terminals/pins are damaged
 
DLC Bus (+) circuit is open or shorted to ground
 
DLC Bus (+) circuit is shorted at an associated module
 
DLC ground circuit is open or has a high resistance condition
 
PCM power relay is damaged or has failed (no power to DLC)
 
Incorrect Self-Test procedure followed, or idle speed to low
 
PCM has failed (e.g., the PCM VREF may be out-of-range)

MAF sensor continuity problems at the connector or harness

Low fuel pressure or vehicle driven until it was out of fuel
 
PCV system has air leaks at the valve or related hoses

HO2S-12 Lack of Switching, Indicates Lean (Self-Test)

Possible Causes:



Air leaks located after the MAF sensor
 
Base Engine: Check for vacuum leaks with a Smoke Tester
 
HO2S wiring may be crossed at the connector
 
Check intake air system for leaks, obstructions and damage
 
EGR system malfunction (the EGR valve may be stuck open)
 
PCV system has air leaks at the valve or related hoses
 
Fuel pressure is too low (fuel filter is dirty, or weak fuel pump)
 
Vehicle driven until it was out of fuel
 

Trouble Code: P1138



HO2S-12 Lack of Switching, Indicates Rich (Self-Test)

Possible Causes:



Check air cleaner element, air cleaner housing for blockage
 
Fuel pressure regulator is leaking, or fuel pressure is too high

Fuel injectors leaking or contaminated (they may be sticking)
 
EVAP vapor recovery system has failed (canister full of fuel)
