Torque Converter Clutch System Malfunction

Possible Causes:



ATF level is too low, or the ATF is badly deteriorated
 
A/T line pressure is low, or the oil pump has failed
 
A/T control valve has failed (it may be stuck)
 
TCC solenoid valve or pressure control valve is stuck
 
TCC in the torque converter is slipping (mechanical fault)
 
TCC shift valve or converter relief valve is stuck
 
Pressure modifier or the pressure regulator valve is stuck
 
Solenoid reducing valve is stuck (mechanical fault)
 
PCM has failed

Torque Converter Clutch Control Engagement Error

Possible Causes:



Brake switch is damaged, out of adjustment or has failed
 
Engine performance conditions are erratic or unstable
 
Engine or transmission temperature is too high

Low fuel pressure or vehicle driven until it was out of fuel
 
PCV system has air leaks at the valve or related hoses

HO2S-11 (Bank 1 Sensor 1) Indicates A/F Ratio Too Lean

Possible Causes:



Air leaks located after the MAF sensor
 
Base Engine: Check for vacuum leaks with a Smoke Tester
 
Intake air system is leaking, obstructed and damaged
 
Low fuel pressure or vehicle driven until it was out of fuel
 
PCV system has air leaks at the valve or related hoses
 

Trouble Code: P1137



HO2S-12 Lack of Switching, Indicates Lean (Self-Test)

Possible Causes:



Air leaks located after the MAF sensor
 
Base Engine: Check for vacuum leaks with a Smoke Tester
 
HO2S wiring may be crossed at the connector
 
Check intake air system for leaks, obstructions and damage
 
EGR system malfunction (the EGR valve may be stuck open)
 
PCV system has air leaks at the valve or related hoses
 
Fuel pressure is too low (fuel filter is dirty, or weak fuel pump)
 
Vehicle driven until it was out of fuel
 

Trouble Code: P1138



HO2S-12 Lack of Switching, Indicates Rich (Self-Test)

Possible Causes:



Check air cleaner element, air cleaner housing for blockage
 
Fuel pressure regulator is leaking, or fuel pressure is too high
 
HO2S wiring may be crossed at the connector
 
Fuel injectors leaking or contaminated (they may be sticking)
 
EVAP vapor recovery system has failed (canister full of fuel)
 

Trouble Code: P1169



HO2S-12 (Bank 1 Sensor 2) Circuit No Inversion

Possible Causes:



An air leak that is located after the MAF sensor
 
Base engine mechanical fault affecting one or more cylinders
 
EVAP Purge valve stuck or the hoses to the valve are reversed
 
Fuel metering fault (fuel injector sticking open or leaking)
 
Fuel pressure too low or too high, fuel supply contaminated
 
HO2S element is contaminated or the HO2S heater has failed
